Filing A Claim

In the event of injury or illness, the Insured should :

1. Visit the campus Student Health Center

2. Present your insurance ID card to the doctor and follow doctor's instructions.

3. Complete a Claim Form for the appropriate company and attach all itemized bills, statements and receipts and mail to the Claims Division specified on top of the claim form.

You can also scan all necessary forms, bills, receipts and statements and e-mail them to the claim department at student-insurance@mcoa.com
